There also exist a small number of copies in hand made sleeves. Included is a one sided card stock insert with images and discographies for all artists, plus a sheet with notes on AMM by Eddie Prévost. The portrait used for Nurse With Wound on the insert is that of John Cage. "You don't need to call it music, if the term shocks you." (John Cage) Label entitles LP "An Afflicted Mans Musical Box". Printed in an edition of 500 copies with this cover.
